The Community Health Engagement Programme (CHEP), established under TTSH’s Continuing & Community Care Department of TTSH in 2009, is dedicated towards empowering the seniors in an active and fulfilling lifestyle.
All programmes, managed by TTSH’s team of healthcare staff (including Doctors, Nurses, Physiotherapists and Occupational Therapists) are suited for seniors from all walks of life, and they are specially designed to be safe, simple and fun. As CHEP is committed to improving the quality of lives of the seniors, the team takes these programmes to the community.
Objectives
- CHEP equips the residents in the community with the skills to monitor and train one another in healthy ageing and self management of chronic disease with the aim of keeping residents in the community as long as possible.
- CHEP aims to integrate TTSH with the community services in the specific geographic locations so as to ensure smooth transition of care of the residents in the community.
Mission
CHEP is the advocate for empowering the seniors in Singapore in taking charge of their health. Together with the network of valued partners, CHEP aims to equip the seniors with the skills needed for an independent and healthier life. This is achieved in an effective and sustainable manner, though education, training and participation from the community
