Introduction

The Department of Palliative Medicine was established in Nov 2008, 12 years after the Palliative Care Service (PCS) was first started by A/Prof Pang Weng Sun, under the Department of Geriatric Medicine. It was the first acute hospital-based palliative care service to be set up in Singapore.

The Palliative Care Service comprises of a multi-disciplinary team of doctors, nurses, social workers and pharmacists working together to improve the quality of life of patients diagnosed with life-limiting illnesses such as advanced cancers and organ failure. Physical, psychosocial and spiritual issues which commonly affect this group of patients and their families are identified and managed holistically.

Over the years, the PCS saw a steady increase in the demand for palliative care in the hospital. In 2008, the PCS attended to more than 1100 new patients, a 15% increase as compared to the previous year.

Services

The Department of Palliative Medicine has a complement of 10 inpatient beds. Besides looking after the patients admitted directly under the department, the PCS also provides inpatient consultative service to the other disciplines. It also runs the Palliative Care Clinic (PCC) which operates daily from Monday to Friday. The clinic sees new patients referred by doctors both within and outside TTSH, as well as patients discharged from the hospital.

The PCS also works closely with the community hospice organizations, providing visiting consultancy service to both Assisi Hospice and Dover Park Hospice.

Besides clinical work, the staff of PCS also contribute actively towards the activities of the Singapore Hospice Council and the education of undergraduate and postgraduate nurses, allied health workers and doctors.

Operational guidelines of the TTSH Palliative Care Service:
1. The PCS consists of an interdisciplinary team that aims to alleviate physical, emotional, spiritual and social pain.
2. The PCS is open to all patients with life-limiting diseases and their families.
3. The PCS works in collaboration with the primary team which refers the patient.
4. The PCS works in close association with community services in order to provide a coordinated programme of palliative and supportive care to patients and families from time of referral through bereavement.
5. The PCS is committed to continuous assessment and improvement of the quality and efficiency of its services.
