Introduction

The Department of Rheumatology, Allergy & Immunology started as the Department of Medicine IV focusing on respiratory disorders and internal medicine. After Prof Feng Pao Hsii, currently the Department’s Emeritus Consultant, took over headship in 1979, the emphasis on management of rheumatic diseases gradually increased. The Department soon became the main referral centre for rheumatic diseases in Singapore. Thus, in August 1995, it was renamed the Department of Rheumatology & Immunology. A new subsubspecialty service, Clinical Immunology / Allergy (CIA) was added in 1988 upon the return of Prof Chng Hiok Hee from her HMDP training. The growth of this new service led to yet another name change to reflect the clinical services provided by the Department. On 1 January 2001, the Department was renamed Department of Rheumatology, Allergy & Immunology (RAI).

The Department presently has 4 senior consultants, 4 consultants, 1 associate consultant and 3 registrars. They are supported by 2 clinical immunology/allergy and 2 rheumatology nurse clinicians

What We Do

The Department of Rheumatology, Allergy and Immunology is a referral centre for patients with rheumatologic and allergic disorders in Singapore and the region. Our doctors manage over 30,000 outpatient attendances including more than 2,000 new referrals each year. The department is a leader in undergraduate and postgraduate training as well as research in Rheumatology and Clinical Immunology /Allergy.

Services

Rheumatology Services

Rheumatologic disorders affect the muscles, bones and joints. Some of these disorders may affect major organ systems like the kidneys, nerves, heart and lungs.

Arthritis

Arthritis is one of the most common rheumatologic disorders seen at our centre. These include rheumatoid arthritis, ankylosing spondylitis, psoriatic arthritis, osteoarthritis, gout and infective arthritis.

Connective Tissue Diseases

Systemic lupus erythematosus (SLE) is the most common disorder in this category that we manage. Others include anti-phospholipid syndrome (APS), Sjogren’s syndrome, scleroderma, dermatomyositis/polymyositis and systemic vasculitides.

Myofascial pain/soft tissue rheumatism

These are common problems often related to workplace and home ergonomics.

Osteoporosis and metabolic bone diseases

These may lead to fragility fractures of the hip and spine in postmenopausal women and the elderly.

Rheumatology Clinics

New patients may be referred to:

Consultant’s Personal Clinic - All named or private referrals may be made to any of our consultant rheumatologists.

General New Case Clinic - Referrals for any suspected rheumatic disease.

Urgent New Case Clinic - Referrals that need attention within 1-2 weeks.

Osteoporosis Clinic - Referrals for established osteoporosis as shown on bone mass density imaging (DEXA scan) of the hip  and lumbar spine with/without fragility fractures.

Stable patients are internally referred by our rheumatologists for continuing care to the following clinics:

  • Lupus Clinic
  • Rheumatoid Arthritis Clinic
  • Spondyloarthropathy Clinic
  • Connective Tissue Disease Clinic
  • Mixed Connective Tissue Disease/Scleroderma Clinic
  • Gout Clinic

  • Musculoskeletal Ultrasound Service

    This is run by rheumatologists trained in musculoskeletal ultrasonography. It provides state-of-the-art diagnosis and treatment of patients with arthritis and soft tissue rheumatism.

    Nailfold Video Capillaroscopy Service

    Nailfold Video Capillaroscopy Service provides the assessment of the presence of microvascular alternations in autoimmune connective tissue diseases.

    Day Treatment & Diagnostic Centre

    This centre provides ancillary outpatient services including:

  • intravenous cyclophosphamide infusion.
  • infusion of biologic agents for refractory arthritis.
  • minor procedures, example: minor salivary gland biopsy, soft tissue and joint aspirations/injections.

  • Where appropriate, patients are referred to the following services available within TTSH:

  • Disease-specific education programme/group therapy run by Rheumatology Nurse Clinicians.
  • Physiotherapy.
  • Occupational therapy.
  • Podiatry.
  • Orthopaedic surgery.
  • Acupuncture service.
  • Pain Clinic.

  • Clinical Immunology/Allergy Services

    We evaluate and manage older children and adult patients with the following conditions:

  • Airway allergy.
  • Allergic rhinosinusitis/conjunctivitis.
  • Asthma.
  • Anaphylaxis.
  • Atopic dermatitis/eczema syndrome.
  • Drug allergy.
  • Food allergy and intolerance.
  • Insect venom allergy.
  • Urticaria and angioedema.
  • Primary immunodeficiency diseases.

  • Clinical Immunology Allergy(CIA) Clinics

    New patients may be referred to:

  • Consultant’s Personal Clinic.
  • All named or private referrals may be made to any of our consultant clinical immunologist/allergists.
  • CIA New Case Clinic.
  • These are for unnamed referrals.
  • Drug Allergy Clinic.
  • Referrals for evaluation and management of drug allergies.
  • Day Treatment & Diagnostic Centre

    The following diagnostic and therapeutic services are available:

  • Skin tests for allergens (e.g. drug, food, insect, aeroallergens).
  • Rhinolaryngoscopy.
  • Provocation/challenge tests (e.g. to food, drug).
  • Drug desensitisation.
  • Specific immunotherapy (for insect venom allergy, allergic rhinitis and asthma).
  • Monthly intravenous immunoglobulin replacement (for antibody deficiency).
  • Education and counselling is provided by the Clinical Immunology/Allergy Nurse Clinician. These include allergen avoidance, emergency management of anaphylaxis (including epinephrine autoinjector use), use of inhalers and skin care for atopic dermatitis.

    Clinical Immunology Laboratory

    Our Clinical Immunology Laboratory provides diagnostic tests for rheumatologic, immunologic and allergic diseases. These include autoantibody tests, serological tests for syphilis, HLA-B27, total and specific IgE to various aeroallergens and food allergens. It is accredited by the College of American Pathologists (CAP) and United Kingdom National External Quality Assessment Schemes (UK NEQAS).

    Immunology Research Laboratory

    This laboratory is funded through research grants from National Medical Research Council (NMRC), Biomedical Research Council (BMRC) and other smaller funding sources. Our main research interests are SLE, rheumatoid arthritis and drug allergy.
