Jointly organised by Tan Tock Seng Hospital (TTSH)’s Centre for Health Activation in partnership with TTSH Central Health, National Healthcare Group, National Healthcare Group Polyclinics, Institute of Mental Health and Yishun Health, the Singapore Patient Action Awards (SPAA) is created to acknowledge individuals and groups who have made significant and notable contributions in enhancing the healing journey of our patients – from partnering in their care journeys to give them hope and encouragement to overcoming their illnesses and getting a new lease of life.

This award honours the caregiver who has demonstrated strength, resilience and unwavering dedication in caring for another person who requires support physical, mental and/or social well-being, amidst health and/or social care challenges. The caregiver takes on the role as an active care partner with the health and/or social care team, to overcome these hurdles with a positive and resilient attitude.

This award is open to any person (including a family member) who provides care and assistance to another person who requires physical/mental/social support (e.g. child, spouse or partner, relative and/or neighbour).

Your nominee should embody the following traits:

  1. Demonstrated exceptional courage, strength and/or resilience in the face of challenges.
  2. Is an active partner of the health and/or social care team in offering constructive feedback to improve care delivery so that other patients will also benefit from it.

This award honours an individual who is very passionate about advocating meaningful causes and has contributed significantly to improving care delivery in partnership with the health and/or social care team/institution. Being personally involved in the care journey as a patient, a volunteer and/or a caregiver, this individual uses his/her personal experience to provide valuable learning for health and/or social care professionals.

This award is open to any person who is involved in the care journey as a patient, volunteer and/or caregiver.

Your nominee should embody the following traits:

  1. Made significant contributions to health or social care delivery.
  2. Led the way to improve health or social care delivery and service.
  3. Appointed as a patient’s dedicated caregiver and/or chaperone:
    1. Empowered to find solutions for treatment plans together with health and/or social care professionals.
    2. Actively educates the patient about his/her condition and keeps the patient and the family informed about the latest treatment plans and developments.

This award honours an outstanding patient support group or volunteer group that has led the way in helping others in need and has contributed significantly to improving health and/or social care delivery.

This award is open to any patient support group or volunteer group which is registered under a healthcare institution, a social service agency, a community care provider or a step-down care establishment.

Your nominee should embody the following traits:

  1. Led the way in helping others in need.
  2. Initiated significant improvements in health and/or social care delivery.
  3. Showed sustainability of these improvements in terms of their duration.
  4. Is an active and established partner with any health and/or social care team/institution to bring sustainable benefits to patients, caregivers and/or residents.

This award is open to any project or initiative that has active participation from members of the public, patients, caregivers and/or volunteers, together with the health and/or social care team.

Your nominee must meet the following criteria:

  1. Project or initiative has delivered improvement(s) to care delivery and overall well-being of patients, caregivers, residents and/or the community.
  2. Collaborated with the healthcare team, intermediate and long-term care teams (ILTC) and/or social service agency.
  3. Showed sustainability of the project or initiative in terms of its duration.

This award honours the work of successful neighbourhood-based initiatives that have contributed significantly to the improvement of care and overall well-being of residents and the local community/neighbourhood.
This award is open to any neighbourhood-based project or initiative that has joint active participation from members of the community (e.g. neighbour, resident, patient, caregiver and/or volunteer).

Your nominee must meet the following criteria:

  1. Community members work closely with community-based agencies to take ownership of the overall well-being of a neighbourhood and helping others in need.
  2. Community-based initiatives can deliver services and represent communities to address their care needs and/or improve the health and/or social care of a neighbourhood.
  3. The community exhibits traits of a ‘Kampung spirit’ as a community of carers and are activated to better care for themselves.
