Since our founding in 1844, TTSH has had roots in the community – being supported by the community and supporting the community in return.
As Singapore’s healthcare needs evolve, our mission is expanding alongside it: to serve patients who come to the hospital, but also to care for residents living in the Central population zone.
To do this, we are bringing together partners across Central Singapore in an integrated model and network of care, built to support residents’ health and social care needs as they age meaningfully within their communities.
Together, we seek to build Healthier, Happier Communities, where people are activated and supported to live well and age well in their homes and communities.
Community Health Teams
Community Health Teams are multi-disciplinary teams embedded in the seven subzones that TTSH serves. Each CHT works with partners in their subzone to co-develop care plans for residents, as well as operate
Community Health Posts located within partners’ premises.
