Advance care planning (ACP) is the process of eliciting a person's values and wishes in the event they are seriously ill. It is founded on the principle of precedent autonomy as well as shared decision making. Every person has the right to make decisions with regards to future healthcare in the event he or she loses mental capacity.
This conversation is difficult but neccessary, and is currently facilitated in all re-structured hospitals. However, there is a need to move the conversation out of the healthcare setting to the dinner table at home.
This talk sheds light on Singapore's journey in ACP, the evidence base of local practice, implementation efforts as well as future directions.
