Family doctors play an important role in the management of our patients’ health in the community. They have a wide breadth of knowledge and establish long-term relationships with patients, and are often the first point of contact for people who require medical advice.

Have you ever wondered what type of care would you prefer in the future should you become very sick and is unable to make health care decisions? If so, get your Advance Care Planning (ACP) done today!  

ACP is a voluntary process of discussion on future care preferences between an individual, his or her family and healthcare providers.
