If you use your Medisave, MediShield Life or Private Medisave-approved Integrated Shield Plans (PMIs) to pay your hospital bills, you are required to sign a
Medical Claims Authorisation Form (MCAF). There are two types of MCAF forms:
- MCAF - Multiple (M) allows you to use your MediSave at all MediSave accredited healthcare institutions. MCAF-M provides convenience as you only need to sign once to use your MediSave funds for life
- MCAF - Single (S) allows you to exercise flexibility should you decide to provide limited consent to use your MediSave funds. You can also limit the type of schemes you would like to tap on, and choose the duration for the usage of your MediSave funds

- For Billing Enquiries, under “Category of Enquiry”, please select "Payment Enquiry" for Change of Depositor Name and
Offsets Request, "Payment Receipts" for Cash/Cheque Transfer, and "Refund" for Refund Requests.
- You can authorise the participating healthcare institutions to withdraw from your Medisave and claim from your MediShield Life/Integrated Shield Plan for approved treatments that you receive at the healthcare institutions. Please follow the steps below to provide consent:
- Login to your HealthHub app using your Singpass
- Tap on “Services” at the bottom navigation bar
- Tap on "Payments & Financials" followed by "Financial Consents"
- Under “MCAF (M) – Medical Claims Authorisation Form (Multiple)”, click on “Give Consent”
Note that authorisation would be effective from date of successful submission. To activate use of Medisave/MediShield Life or both for your hospital stay, please approach our financial counsellors.
