ESD is a home-based rehabilitation programme for patients who are medically suitable for discharge from the acute hospital but require rehabilitation. We link inpatient care with community rehabilitation services, to allow patients to be discharged home sooner with support of the community rehabilitation team. Patients should not have high care needs, and should have an available caregiver who must be deemed to be capable by our therapists and nurses.

ESD empowers patients to be more self- reliant, and to be proactive in their own recovery. Patients feel more confident in moving about their own home and doing things independently.

The ESD team is composed of different health professionals and therapists – closely working together with the case manager overseeing the patient's progress and journey into recovery. As a team, the patient's progress and rehabilitation goals are discussed. The case manager then assists to follow up on plans for each patient.

The ESD home based rehabilitation programme caters to patients with mild to moderate stroke, non- stroke neurological conditions, musculoskeletal conditions, and hospital acquired deconditioning. These patients will be assessed by the Rehabilitation Medicine physician for clinical suitability and for rehabilitation goal setting.
