​Overview

The Department of General Medicine provides comprehensive and holistic care for patients with a range of simple to complex medical conditions, and coordinates diagnostic work-up of undifferentiated symptoms and management of multi-system illnesses.

We work closely with other Specialists, Nursing and Allied Healthcare workers to provide timely care and interventions for our patients.

Our department also runs the Acute Medical Unit for short stays, Mobile Inpatient Care At Home (TTSH@Home), and subacute care for patients who need further rehabilitation. We also actively provide inpatient consult and outpatient clinic services in the areas of General Medicine, Vascular Medicine, Peri-operative Medicine, Hypertension and Obstetric Medicine.

Our department provides consultations on:

  • Work up of undifferentiated symptoms such as recurrent falls, loss of consciousness, confusion, giddiness, loss of appetite, weight loss, persistent fever, low blood count and lower leg swelling
  • Simple and complex infections
  • Investigation and management of blood clots
  • Holistic management of complex chronic disease, salt and metabolic disorders - for example: Hypertension, Diabetes Mellitus, Chronic Kidney Disease, thyroid conditions, electrolyte imbalances etc
  • Management and discussion of end-of-life care and Advance Care Planning​

Our Services

TTSH Mobile Inpatient Care at Home (TTSH@Home)

The ​Tan Tock Seng at Home (TTSH@Home) service comprises a dedicated team of General Medicine doctors and nurses, who are able to provide definitive inpatient care for patients in the comfort of their own home. Following a referral of suitable patients to TTSH@Home, the logistics required to care for them at home will be arranged, together with daily telemedicine consultations and/or house-calls to continue appropriate investigations and treatment in their home setting.

Hypertension Clinic and Blood Pressure Service

Our Hypertension Clinic specialises in the investigation and management of high blood pressure, in particular, the evaluation of secondary causes of hypertension as well as management of difficult to control blood pressure across all age-groups. We also have a pharmacist-led Blood Pressure Service, which plays a supportive role by expediting blood pressure control through home blood pressure monitoring and tele-health management. 

General Medicine Complex Care Clinic (GMCC)

Our General Medicine Complex Care (GMCC) clinic manages patients with multiple complex medical conditions, who would benefit from comprehensive oversight under one primary physician. In partnership with the GMCC clinic, we also have a pharmacist-led medication review clinic which supports our patients by reviewing their medication regimen, and monitoring their adherence and tolerance to multiple medications.

Vascular Medicine and Anticoagulation Clinic

Our Vascular Medicine service provides both inpatient and outpatient expertise for preventing and management of blood clots and other chronic conditions affecting the veins, arteries and lymphatic system.

These conditions may include:

  • Deep vein thrombosis (blood clot in the legs)
  • Pulmonary embolism (blood clot in the lungs)
  • Chronic venous insufficiency (eg varicose veins and leg swelling)
  • Peripheral artery disease (narrowing of leg arteries)

Our Anticoagulation Clinic (ACC) is run by pharmacists and offers counselling and monitoring when patients require anticoagulation (blood thinner medications), and includes peri-procedural management of anticoagulation.

Peri-operative Medicine

The Perioperative team within General Medicine runs daily clinics at Clinic 5A and the PACE (Preoperative Counselling and Evaluation) Clinic in B2B. Working in close conjunction with Anesthesia and General Surgery, we optimize medical conditions and medications for patients prior to planned elective procedures, so as to achieve the best possible outcomes.

Advance Care Planning (ACP)

The Department of General Medicine hopes to empower our patients to “Live well & Leave well” through Advance Care Planning. Advance Care Planning allows our patients to share their healthcare priorities and be cared for holistically, and can be done during the inpatient stay and/or outpatient follow up.
