Our Community Health Team (CHTs) services include:
- Health checks;
- Health education and health monitoring;
- Health coaching and activities;
- Nursing care;
- Medication advice;
- Coordination and referrals to other health and social services.

Charges and Subsidies
Our services are chargeable and you will be required to undergo a Means-Testing Assessment. Additional charges may be included for laboratory tests, procedures and/or medical supplies.
All charges are not payable by Medisave or other financial assistance schemes (including CHAS, PG and CSC).
We will refer you to our Medical Social Worker for a financial assessment should you require further assistance.
