Chong Hua Tong Senior Activity Centre (SAC)

​​​To better manage frailty needs and delay frailty progression, Chong Hua Tong SAC has co-planned a series of screenings, interventions and awareness raising activities that were rolled out in February 2023 in collaboration with the Community Health Team (CHT)’s nurses, Health Coaches and dieticians:

  • A Community Nurse Post (CNP) was started to increase accessibility of care services in the community. Clinical services available include: conducting geriatric assessments, addressing issues of non-compliance to treatment plans, providing healthcare supervision, coordinating resources to meet psychosocial needs and working with partners to monitor work plans.
  • In February 2023, a functional and falls screening was also conducted to identify residents at risk of adverse health outcomes in terms of falls, admission to hospital, the need for long-term care, etc. Post-screening, residents with high and moderate risk for falls were right-sited to the CNP and group coaching programme by CHT Health Coaches respectively to combat frailty.
  • ​In the same month, as part of 2023’s Dietician Day initiatives, CHT dieticians rallied for TTSH staff to donate a list of curated foods with plant-based proteins to support CHT SAC. Seniors could earn stamps for food items through participation in the centre’s activities, as part of Chong Hua Tong’s food redemption programme. To guide seniors as they shop and redeem items from the nutritious food supply from the mini mart, post-delivery of the rations, CHT dieticians labelled the rations with stamp tags and pictorial facts, and arranged the food items onto assigned shelves to educate seniors of the Chong Hua Tong area on nutrition and the healthier choices available for healthier and stronger bones.
