Frailty Management @ Care Corner

​​​​Care Corner in Toa Payoh recognises the need for targeted,systemic and research-based interventions to better provide care to residents in the community.

As such, Stronger Together, a holistic frailty management programme was conceptualized by Programme Executives, Wellness Trainers from Care Corner and with inputs from the TTSH Community Health Team’s (CHT) physiotherapists and dieticians. Stronger Together onboards and screens seniors aged 60 years and above for risk factors in the domains of physical function, nutrition and psychosocial well-being.

​While the team recognises the inevitability of frailty, experience has also proven that frailty can be delayed through early intervention. Stronger Together aims to facilitate the detection and delay of frailty onset through progressive and individualized physical activities, resistance training, balance training, and nutritional enrichment and education. While Care Corner executes, data collects and analyses the effectiveness of the programme, TTSH CHT contributes by providing training, advice, and validating the programme assessment and intervention tools.

Recognizing that every individual is unique, a person-centred approach forms part of the core of the programme, and care goals and plans are discussed and co-developed with the seniors. Involving seniors in the care process motivates and builds commitment towards their care goals to maintain their health and to live their golden years to the fullest. As of today, Stronger Together has been piloted at 3 Toa Payoh sites with plans for spread post-evaluation.
