As Singapore's population ages, older adults may face challenges with memory, difficulty in ambulation and falls leading to a decline in function.
Our dedicated team of Nurse Clinicians together with the multidisciplinary team provide specialised care and support for individuals aged from 65 years and their families. The focus of care will be on early identification and intervention to enable the older adults to maintain independence and quality of life while delaying the onset of frailty.
- Individualised Interventions: Evaluation for their referred problem with a comprehensive focus on Function (ability to perform Activities of Daily Living), Mobility and Cognition. They also look into the Psychosocial and Environmental factors that may affect the above outcomes.

Caregiver Education and Support: Providing educational resources and counselling on Dementia care, Falls prevention, Nutrition and Continence care, including pressure injury prevention, and caregiver well-being.

Community Integration: Collaboration with community healthcare partners and referrals to community-based services as and when needed.

Geriatric Assessment Clinic (GRAC): Provides comprehensive evaluation and management for older adults experiencing multiple geriatric syndromes or recent decline in their ability to perform daily activities.

Cognitive and Memory Disorders Clinic: Provides comprehensive evaluation and management for older adults with memory impairment and related problems.

Fall and Balance Clinic: Provides comprehensive evaluation and management for older adults with recurrent falls and gait problems.
