​​​​​Jamie Lim

Jamie was certified as an Advanced Practice Nurse (APN) in 2006 and became TTSH’s first APN. She played a key role in establishing the APN framework and took on leadership in 2012 to build the foundation for the profession. In 2017, she began overseeing 55 Nurse Clinicians (Specialty), and the clinical team has since grown to over 100, supported by two APN-ADNs. A passionate clinician-educator, Jamie has been actively involved in training nursing, medical, and allied health professionals, both locally and in rural communities across Thailand, China, and the Philippines. In 2022, she became a certified Collaborative Prescriber after completing the National Collaborative Prescribing Programme and now chairs key committees that ensure safe prescribing in TTSH. That same year, Jamie joined the Nursing Education Department (ELITE), where she continues to blend her love for clinical work and education in driving innovation and excellence in nursing practice.

Jennifer Wong Chee Mei

Jennifer Wong is an Advanced Practice Nurse with the Heart Failure (HF) Service, Cardiology at Tan Tock Seng Hospital (TTSH). She obtained the Master in Nursing ​in 2010 and is a certified prescriber, having achieved her certification from the National Collaborative Prescribing Programme in 2021. Jennifer began her career in the intensive care unit in 1992 before leading the implementation of a multidisciplinary Heart Failure Disease Management Programme in 2002. Her dedication led to the launch of an APN-led HF clinic in 2012. A passionate advocate for HF care, she contributed to the Singapore HF clinical guidelines published in 2020, actively shares her knowledge at regional and local conferences, and supports research and quality initiatives to standardise and improve HF care. She also serves on the Service Review and Credentialing Committees at TTSH and mentors nursing students. In addition, she is a faculty member at the Lee Kong Chian School of Medicine and leads a team of cardiac nurses to deliver evidence-based care for patients with heart failure.

​​​​Jiang Yan

Ms Jiang Yan is a practicing Advanced Practice Nurse in Cardiac Intensive Care Unit and Assistant Director of Nursing for Nursing Specialty. She is particularly interested in hemodynamic monitoring and post cardiac arrest syndrome care. She also oversees the nurse clinicians specialty.

She is currently a honourery lecturer in Alice Lee Centre for Nursing Studies, National University of Singapore.

Samuel Soon Guo Yu

Samuel Soon Guo Yu is an Advanced Practice Nurse in the Department of Cardiology at Tan Tock Seng Hospital. He holds a Bachelor of Science (Nursing) and a Master of Nursing from the National University of Singapore. With over six years of experience in medical-cardiology and acute care, he has led quality improvement initiatives and mentored junior nurses, earning the NHG Teaching Award in 2019. Licensed as an APN in 2023, he completed the collaborative prescribing program in 2024. Samuel currently manages cardiac patients under the Heart Failure Service in both inpatient and outpatient settings. He also sees patients with early cardiac symptoms in the Cardiac Stress Investigation Clinic. Passionate about improving patient outcomes, he advocates for healthy lifestyle changes and evidence-based care.

Rina Ng

​Rina ​is an Advanced Practice Nurse (APN) in Cardiology at Tan Tock Seng Hospital, with a special interest in Pulmonary Hypertension. She has worked in the cardiology general ward since graduating with a Bachelor of Science (Nursing) from the National University of Singapore in 2014. She holds a Diploma in Nursing (2012), an Advanced Diploma in Nursing, Gerontology (2019), and a Master of Nursing (2023), during which she received several academic awards including the Lee Foundation Medal, Libby Tin Peh Medal, and Wee Kim Wee Medal. In 2024, she completed her APN internship, obtained full accreditation, and completed the Collaborative Prescribing Practitioners Programme.
