​​​​​Jasmine Kang

Ms Jasmine is the Deputy Director of Nursing at Tan Tock Seng Hospital with over 20 years of experience. As Singapore’s first Geriatric​-trained Advanced Practice Nurse (APN), she was instrumental in establishing the nation’s first delirium​ unit—the Geriatric Monitoring Unit (GMU). Jasmine holds a Bachelor of Health Science (Nursing), a Diploma in Gerontology Nursing, and a Master of Nursing, and became a certified APN in 2009. She leads Population Health (Nursing) for the Central Health cluster and co-leads the Frailty Ready Hospital initiative, supporting seniors across all stages of ageing. Clinically, she heads the GeriCARE team to provide geriatric consultations for frail inpatients. From 2019 to 2023, she was seconded to the Ministry of Health to develop the National Integrated APN Internship. A passionate educator and mentor, Jasmine was honoured with the President’s Award for Nurses in 2013 for her contributions to advancing elderly care.

Ng Wan Ring

Ms Wan Ring is an Advanced Practice Nurse (APN) specialising in geriatric care, with over 13 years of experience. She holds a Master of Nursing from the National University of Singapore and became a certified APN in 2022. In 2024, she obtained her collaborative prescribing licence.

At Tan Tock Seng Hospital, she is part of the Emergency Department Interventions for Frailty (EDIFY) service, which delivers early geriatric input in the Emergency Department to reduce unnecessary admissions. She also draws from eight years of experience in the Geriatric Monitoring Unit, caring for patients with delirium or dementia and mentoring fellow nurses.

​Dedicated to advancing age-friendly care, Ms Wan also serves as a course coordinator with the Institute of Geriatrics and Active Aging​​, supporting the training of healthcare professionals in dementia care.

Wang Xia Xia

Ms Wang Xiaxia is an Advanced Practice Nurse (APN) in Geriatric Medicine at Tan Tock Seng Hospital. She holds a Master of Nursing from the National University of Singapore and has been in nursing since 2007. Specialising in the care of older adults, Xiaxia is based at the Integrated Care Hub (ICH), where she provides holistic, individualised care for older persons with dementia and frailty. She conducts comprehensive geriatric assessments and leads care planning tailored to each patient’s needs. Passionate about nursing education, she actively trains nurses and spearheads clinical initiatives within the ICH. Since 2022, she has also served as a Clinical Practice Improvement Programme (CPIP) facilitator with the National Healthcare Group.
